Abstract

Literature has historically supported the discussion surrounding pharmacologic management of the pregnant patient who abuses opioids to include methadone or buprenorphine, but it has not provided as comprehensive of a review to compile that information with specific goals for counseling during the intrapartum and postpartum period. This review seeks to enhance prior dialogue on counseling by recognizing the various social determinants of health affecting this patient population, as well as patient-specific goals to encourage sobriety. It will also expand on recommended protocols for initiating pharmacotherapy and appropriate drug selection. Furthermore, various screening tools for opioid abuse are mentioned and assessed for sensitivity and specificity.
